This form is ONLY for Primary Club Advisor or an Associate Club Advisor the Primary has sent this to so they may request a Everyday Season Pass for their with the School Club or other personal visits.

Only Primary Club Advisors may request Generic Chaperone Once-A-Week Passes. These are used on a week to week basis so the Club Advisors may give skiing privileges to those assisting with the club. These people may be other educators, parents or friends. These passes do include equipment rentals if needed and can ONLY be utilized on the day a club is scheduled to transport. 

Please include below how many Generic Chaperone Passes you believe you may need to assist your club for this season. You can request more at a later time. We offer no guideline on what you may request because all clubs have different requirements and ages of students. We do reserve the right to limit those requests we believe exceed our expectations.

PASS PHOTO UPLOAD: You are welcome to send us a photo for the Passholder in the format of JPG or JPEG. The size is limited too 5MB or approximately 1600x1200 pixels. Please use a photo resizer app if you do not know how to get an image within these requirements. This is not required at this time but helpful in preparing your pass before first visit.

Skiing Classification: *

Note the Skiing Classification for the Passholder. 

Type I: Ski conservatively; prefer slower speeds, prefer easy, moderate slopes; Favor lower then average/retention settings; including all "entry-level skiers uncertain of their classification. This corresponds to an increased risk of inadvertent binding release in order to gain increased releasability in a fall. 

Type II: Ski moderately; prefer a variety of speeds; ski on varied terrain, including most difficult trails; including all skier who do NOT meet all descriptions of either Beginner or Advanced

Type III: Ski aggressively; normal ski at high speeds: prefer steeper and more challenging terrain; favor higher then average release/retention settings. This corresponds to decreased releasability in fall in order to gain a decreased risk of inadvertent binding release.
